As a courtesy to our office, we ask that you please provide us with at least 24 hours notice for cancellations. This allows us to free up your time slot for others who might be waiting for something to open up. No shows or late cancellations will be charged a fee of $30. We understand that life happens and sometimes things are simply out of your control, therefore we reserve the right to waive any fees at our discretion.

For new patients, either plan on arriving a few minutes early to your first appointment to fill out new patient forms, or download them from our website and fill out ahead of time to bring with you.

After you choose a date & time, you will create an account with your details and complete the booking.

Your email address and password will then allow you to review, change and add appointments.
